As a VSP member, you and your covered dependents will get the personalized care you need to take care of your vision and overall health.

These are the periods when you are eligible to enroll for Vision Insurance:

Within your new hire enrollment period
If you have a qualifying life event (you get married, have a baby, etc.)
During your employer’s open enrollment

Who is the provider?

VSP

VSP was founded by a group of optometrists in 1955. More than 65 years later, that vision has evolved into providing world-class products and services to eye care professionals, employers, and more than 80 million members worldwide.

Who in my family is eligible for this coverage?
Members of your family who are considered eligible to enroll for coverage include:
- You – if you are an active part-time Associate
- Your spouse/domestic partner
- Your child(ren) – under age 26

How much does this coverage cost?
Monthly Rates (effective 10/1/24)
- Member only – $6.52
- Member + 1 – $12.40
- Member + family – $18.92

What if my employment status changes?

Upon separation of employment with Ross or dd’s, you may be eligible for continued coverage under COBRA. If you’re eligible, you will receive a COBRA packet from VSP. If you have questions, contact VSP at (800) 400-4569.

When would my coverage start?

If enrolled during open enrollment, your coverage effective date is October 1 of the current year.

For new hires and qualifying life events, coverage is effective the first of the month following enrollment.

How do I submit a claim?
If you’ve seen a VSP network eye doctor or provider, you don’t need to fill out a claim form. The doctor or provider will submit the claim directly to VSP for processing after your appointment. The doctor or provider will discuss any copays or out-of-pocket expenses with you during your appointment.

If you’ve seen an out-of-network provider, your coverage will be less than when you see a VSP network eye doctor. Contact VSP Member Services at (800) 400-4569 for assistance submitting an out-of-network claim. Or submit your claim online by following these steps:
- Login to your VSP member account
- Click on View Your Benefits
- Click Submit a Claim under Oops! Did You Go Out of Network?
- Click Start New Claim

How do I locate an in-network provider near me?

To find a VSP-network doctor near you, log in to your member account and click Find a Doctor in the top of the navigation bar. Enter your zip code or city and state to find a list of in-network doctors and retail locations near you.

Search Tip: Whether your logged in or not, you can use the “advanced search” feature to search by type of doctor, frame brands available, hours and scheduling options, gender, language(s) spoken, and more.

How can I cancel my vision coverage?

You can cancel coverage during the open enrollment period or if you have a qualifying life event. Contact VSP at (800) 400-4569 to make changes.
